The following items are designed to facilitate your meeting the Basic readiness standards with the use of easy to reference information and links.

 

BASIC READINESS LEVEL checklist: (9 STEPS TO READINESS!!!!!)

¸          1.  Physical exam and medical history submitted to the Medical Affairs Branch (page 3, MC377) once every 5 years.

¸          2.  Complete all immunization requirements for Measles, Mumps, Rubella, Varicella, Tetanus/Diphtheria, Polio, Influenza annually, Tuberculosis screening (PPD) annually, and begin the Hepatitis A + B series (both of these series must be completed by May 2005.)  Details of how to meet the immunization requirements:  http://ccrf.hhs.gov/ccrf/ccrf_newsletter_041904.htm#IMMUNIZATIONS

¸                   3.  Complete the 12 defined online training sessions:  http://ccrf.hhs.gov/ccrf/Training_Page_Roll_Out.htm

¸                   4.  Maintain certification in AHA Basic Life Support for Healthcare Providers and enter certification expiration date  by updating your Officer Summary Page after logging in at http://ccrf.hhs.gov/ccrf/Database/login2.htm.  The only acceptable equivalent course is American Red Cross CPR/AED for Professional Rescuers course. (Page 5-6 MC 377).

¸                   5.  Record your height and weight every 12 months by updating your “Officer Summary Page” after logging into the Office of Force Readiness and Deployment (OFRD), formerly Commissioned Corps Readiness Force/CCRF, website at http://ccrf.hhs.gov/ccrf/Database/login2.htm

¸                   6. Meet fitness requirement by:

a) joining the "President's Challenge" at www.presidentschallenge.org

b) Complete the Annual Physical Fitness Test (APFT)-requirements listed at <http://ccrf.hhs.gov/ccrf/physical.htm> then enter your APFT results on the OFRD website after the log in screen AND submit your APFT to MAB on form PHS 7044 (http:\\dcp.psc.gov/DCPForms.asp)

¸                   7.  Maintain a current license to practice dentistry and submit to your OPF and enter the license expiration date on your Officer Summary Page after logging in at http://ccrf.hhs.gov/ccrf/Database/login2.htm.  

¸                   8.  Choose a deployment role and team selection at https://ccrf.hhs.gov/ccrf/OFRD_Officer_Questionnaire.htm and if a clinical Dentist, record the number of clinical hours performed the prior calendar year by logging in to update your Officer Summary Page at http://ccrf.hhs.gov/ccrf/Database/login2.htm.   (Page 6 MC 377)

¸                   9.  Own all required uniforms and enter information on Officer Summary Page.  Report in required uniform when deployed or on a response.  (Page 6 MC 377)
